Sample Collection Appropriate sample containers are provided at no charge. Special charges apply for rush or overnight delivery of samples. Davy Laboratories is not responsible for sample integrity or improper sampling procedures. Analysis cannot be considered valid unless samples are properly collected, preserved and holding times are met. Samples may be refused of the data qualified if they do not appear to have been properly collected and stored. The client may elect to proceed with analysis or resample. Written client information and analytical instructions on a signed and dated chain-of-custody form are required for all samples submitted to the laboratory. Sample Receipt and Documentation Laboratory hours are 8 am to 4:30 pm, Monday through Friday, excluding holidays. Contact the laboratory to arrange sample deliveries at times other than normal working hours. A surcharge may apply for deliveries and analytical services outside normal working hours. Terms Qualified clients may establish an account and be invoiced monthly or upon sample report. Terms are net 30 days, with a 1.5 percent per month interest charge on amounts outstanding. Fees may be paid by credit card (MasterCard or VISA). Minimum Invoicing A minimum invoice of $30.00 will apply, unless prior arrangements have been made. SDWA microbiological analysis for total coliforms and Department of Agriculture swab tests are exempt from the minimum invoice fee. Rush Order Processing When normal turnaround time does not meet your needs, call to arrange for rush order processing of your samples. By providing us with complete information regarding the number of samples and analyses required in advance of delivery, you will help ensure on-time delivery of the results. Rush results are available verbally. A written report may or may not be available immediately. Volume Discounts and Special Pricing Quantity discounts apply to sample batches received at the same time. Special discounts are also available for specific projects and annual contracts. Quality Assurance/Quality Control Reports (QA/QC) We will supply QA/QC reports at no additional charge when the request is made prior to sample analysis. The QA/QC report will provide the following information: precision and accuracy statements, raw data and sample chromatograms, if applicable. QA/QC requests after report preparation but before one year will be billed at 25 percent of the original invoice (minimum of $30.00). Any requests for reports and/or QA/QC information after one year from the date of report will be assessed an hourly fee plus copying costs. Subcontracting Davy Laboratories maintains subcontracting relationships with other laboratories that meet our quality control criteria and have appropriate certifications. We reserve the right to subcontract if needed. Report Formats In addition to standard written reports, we can also fax results, complete your data forms, e-mail results or prepare customized electronic deliverables for specific regulatory programs.
